BENEFITS

Dental

Dental Insurance through Anthem Blue Cross. CR Associates contributes 70% of the Dental premium for the employee and dependents. Employees are eligible for Dental benefits on the first day of the next month after 30 days of employment. 

VISION

Vision Insurance through VSP. CR Associates contributes 70% of the Vision premium for the employee and dependents. Employees are eligible for Vision benefits on the first day of the next month after 30 days of employment. 

401(K) Retirement Plan 

CR Associates offers a Safe Harbor 401(K) Retirement Plan. CR Associates provides an employer match of up to 5% on 6% of employee contribution. Employees are eligible to participate in the 401(K) plan after 12 months and 1000 hours of service. 

Medical  

CR Associates offers Medical Insurance through Cal-Choice Anthem Blue Cross and Kaiser Permanente. CR Associates contributes 70% of the medical premium for the employee and dependents. Employees are eligible for medical benefits on the first day of the next month after 30 days of employment. Employees and their eligible dependents may choose to enroll in the Anthem PPO plans or the Kaiser HMO plans. 

Life and Accidental Death & Dismemberment

CR Associates offers Life and AD&D Insurance through Blue Shield of California. The life insurance benefit amount designated by CR Associates is $100,000 with an optional dependent benefit amount of $5,000. Chen Ryan contributes 70% of the Life and AD&D premium for the employee and dependents. Employees are eligible for Life and AD&D benefits on the first day of the next month after 30 days of employment. 

Flexible Spending Account (FSA) 

CR Associates offers a Flexible Spending Account (FSA) Plan. Employees are eligible for FSA enrollment on the first day of the next month after 30 days of employment.
